Classify data responsibly

Voluntary reports do not automatically represent all treatments or risks. Reporting frequency, the actual incidence of a problem and an organisation’s safety culture are different measures. The report therefore does not present unsupported clinic rankings or purported complication rates.

Reporting structure

• Actual time period, goals and data sources.
• Receipt and processing, reporting distortions and relevant process phases.
• Reviewed, anonymised learning findings and standards development.
• Assessment programme activity and education events actually delivered.
• Completed historical source work and today's learning questions.
• Funding, conflicts of interest, restrictions and next steps.
